Dear Sir/Madam,
I am writing to urgently request a full care needs assessment and appropriate support for my mother, Alaweyah Sulaiman, who is suffering from advanced dementia. Her condition has significantly deteriorated and is now putting her safety and well being of my elderly father at serious risk. Recently my mother was involved in a distressing incident where she attempted to climb over a fence and sustained serious injuries to her right hand, requiring multiple stitches. She has also been forgetting to ash, change her clothes, and has been wetting herself which causes her considerable distress and confusion. I am deeply concerned for their welfare and respectfully urge Social services to carry out an urgent assessment of my mother's needs and provide the necessary support. Be it through regular home care, respite care or consideration for more long term solutions. Please treat this matter as urgent. I am happy to provide any additional information or assist with next steps. Thank you for your time and attention
